Best online weight loss clinic in Spain (2026): what to compare and what to expect
No single online clinic is the best for everyone. What you can do is compare them sensibly. Look for a proper medical assessment, a doctor you can identify, the genuine medicine obtained legally, follow-up, a clear price and a plan that goes beyond the drug. This guide covers what to expect from a clinic, what results the trials show are realistic, and the signs that a clinic isn't trustworthy.

Short answer
In Spain, no single online clinic is the best for everyone. The right one carries out a proper medical assessment before prescribing and has a doctor you can identify. It gives you a prescription to collect the genuine medicine from a pharmacy, follows you up and shows the full price. Patients pay for these obesity treatments themselves, so it pays to compare carefully.
Key points
- SEEDO's GIRO Guideline, the Spanish reference on obesity, was developed with 35 scientific societies and 12 patient associations, and recommends comprehensive, multidisciplinary treatment.
- In the STEP 1 trial, semaglutide produced an average weight loss of 14.9% over 68 weeks, compared with 2.4% on placebo, but only 50.5% of participants lost at least 15%.
- In the SURMOUNT-1 trial, tirzepatide produced an average weight loss of between 16.0% and 22.5% over 72 weeks, depending on the dose.
- In STEP 1, 4.5% of patients on semaglutide stopped treatment because of digestive effects, compared with 0.8% on placebo. That's why follow-up matters.
- The EMA warned of falsified Ozempic pens in the EU, a risk that arises when the medicine is not obtained through the legal route.
How to compare online weight loss clinics
General criteria for judging any service, ours included. They don't replace an individual medical assessment.
| Aspect | What you should find | Red flag |
|---|---|---|
| Initial assessment | A doctor reviews your history, medical conditions and medication, and can say no | A form that always ends in a prescription |
| Who prescribes | A named doctor registered with a Spanish medical association | You don't know who signs the prescription |
| Medicine | A prescription to collect it from a pharmacy, in its original packaging | Direct sale without a prescription, or delivery from a website that isn't a pharmacy |
| Follow-up | Regular reviews and a clear contact if side effects appear | Nobody replies once you've paid |
| Price | Medical service and medicine shown separately, with dose-related changes explained | Fees that don't say what they include |
| Approach | A plan covering habits and realistic goals, as the GIRO Guideline recommends | Promises of weight loss without changing anything |
No single online weight loss clinic is the best for everyone. The right one for you assesses you properly before prescribing anything and tells you who the doctor is. It gives you the genuine medicine through the legal route, follows you up and shows you the full price from the start. If a clinic falls short on any of these points, a slick website makes no difference.
This article is written by AiON, which is itself an online clinic. That's why we're not going to publish a ranking with ourselves at the top. Instead, we give you the criteria you can use to judge any service, ours included.
Why so many online weight loss clinics have appeared
In Spain, the best-known obesity drugs are semaglutide (Wegovy and Ozempic) and tirzepatide (Mounjaro), and they are prescription-only. When they are used to treat obesity, patients pay for them out of their own pocket. In December 2025, Spain's Interministerial Committee on Medicinal Product Pricing decided not to fund Mounjaro for this indication through the Spanish National Health System (SNS), according to Redacción Médica.
High demand, a treatment that patients pay for themselves and the convenience of remote consultations have led to dozens of private services. Some are very rigorous. Others work almost like a shop with a form in front of it. From the outside they look alike, which is why it pays to know what to look for.
The starting point: what the Spanish obesity guideline says
The reference guideline in Spain for treating obesity in adults is the GIRO Guideline. Its 2nd edition was published by the Spanish Society for the Study of Obesity (SEEDO) in late 2024, with input from 35 scientific societies and 12 patient associations. It rests on three ideas: don't stigmatise the person, treat obesity in a comprehensive, multidisciplinary way, and involve the patient in decisions.
This gives you a yardstick. Some clinics simply sell a drug and say nothing about diet, physical activity, sleep, emotional health or your other medical conditions. Even if the drug is legal and effective, such a clinic is far from what the reference guideline recommends.
What to compare between online clinics
A genuine medical assessment, not a form that always says yes
Before suggesting any treatment, a doctor should know your weight and height, your medical conditions, your medication, your medical history and what you've already tried. They should also be able to say no, or tell you that you need a blood test first. If the outcome is always the same whatever you answer, you're not being assessed. You're being sold to.
Knowing who is treating you and where the medicine comes from
It's reasonable to ask for the name of the doctor who signs the prescription and whether they are registered with a Spanish medical association (colegiado). You can also ask how the medicine reaches you. Normally you get a prescription and collect the medicine from a pharmacy, in its original packaging. An offer to send you the drug without a prescription, or from a website that isn't a pharmacy, is a red flag. To understand the differences between the drugs and what each one is authorised for in Spain, see our guide to what Ozempic is and who can take it.
Follow-up and managing side effects
These treatments start at a low dose that is increased over time, and digestive discomfort is common, especially at first. In the STEP 1 trial, 4.5% of people taking semaglutide stopped treatment because of gastrointestinal effects, compared with 0.8% on placebo. A reliable clinic tells you who to contact when something isn't right, how often your progress will be reviewed and what to do if you don't tolerate a dose well. To find out which side effects are expected, read what to expect from Ozempic and Mounjaro side effects and how they are managed.
A complete, clear price
The monthly cost is usually made up of two things. One is the medical service (consultation and follow-up). The other is the medicine, which you pay for at the pharmacy and whose price depends on the drug and the dose. The clinic should show these two items separately and explain how the cost changes as the dose goes up. Be wary of monthly fees that don't say what they include. To work out the figures, see our breakdown of how much Mounjaro costs per month by dose.
A plan for the long term, not just for getting started
Obesity is a chronic disease. A good clinic talks from the outset about habits, realistic goals and what happens if you stop treatment one day. This is one of people's biggest concerns, and we cover it in why weight comes back after stopping treatment and how it's managed.
What results are realistic according to the trials
A reliable clinic explains results with data and nuance, without promising you a figure. The STEP 1 trial enrolled 1,961 adults without diabetes. They had a BMI of 30 or more, or 27 or more with a weight-related condition. Over 68 weeks, with lifestyle changes in both groups, people on semaglutide lost an average of 14.9% of their body weight, compared with 2.4% on placebo. The average hides a lot of variation: 86.4% lost at least 5%, 69.1% at least 10% and 50.5% at least 15%. In other words, half the participants didn't reach 15%.
The SURMOUNT-1 trial enrolled 2,539 adults without diabetes who met similar criteria. Over 72 weeks, tirzepatide produced an average weight loss of between 16.0% and 22.5%, depending on the dose. In both trials, the drug was combined with lifestyle changes. These data don't support a website that promises weight loss without changing anything else, or claims that everyone loses the same amount.
Warning signs to look out for
- They sell you the medicine without a prescription, or the prescription arrives without any doctor having reviewed your case.
- You don't know who the doctor is or how to contact them if something goes wrong.
- The drug arrives by courier from a website that isn't a pharmacy, or in packaging that isn't the original.
- They promise guaranteed results, no side effects, or results without changing anything in your daily life.
- They pressure you with time-limited offers, or the final price doesn't appear until you've already paid.
The risk of getting a fake product is real. The European Medicines Agency (EMA) warned patients and healthcare professionals in the EU about falsified Ozempic pens in the supply chain. When scanned, their serial numbers showed as inactive. In Spain, there have also been reports of illegal online pharmacies selling falsified or unregistered Ozempic. Buying outside the legal route isn't a shortcut. It means not knowing what you're injecting.
Key takeaways and next step
The best online clinic isn't the one that promises the most or the cheapest one. It's the one that assesses you, tells you who is treating you and gives you the genuine medicine on prescription. It supports you as your dose is adjusted and works with you beyond the drug, as the GIRO Guideline recommends. In Spain, that route already exists: medical weight loss treatment with an assessment first, a prescription only when appropriate from a registered doctor, and ongoing follow-up. That's how we work at AiON. You can check in a few minutes whether you're eligible and then decide with all the information.

Frequently asked questions
- What are my options in Spain for medical weight loss treatment without going to a clinic?
- You can see your GP or an endocrinologist, either through the public health system or privately. You can also use a telemedicine service with registered doctors. In every case, obesity drugs need a prescription and an assessment first. The difference lies in convenience and follow-up, not in skipping the medical assessment.
- How much does medical weight loss treatment cost per month in Spain?
- It depends on the drug, the dose and what the clinic charges for follow-up. For obesity, the patient pays for the medicine. In December 2025, funding for Mounjaro for this indication through the Spanish National Health System was turned down. The sensible approach is to ask for an itemised total cost, with the medical service and the medicine listed separately, and to ask how it changes as the dose increases.
- I've been told I have fatty liver. Will losing weight reverse it, and what treatment is available in Spain?
- Losing weight is usually the cornerstone of managing fatty liver linked to excess weight, but how much it improves depends on the individual and the degree of liver involvement. A doctor needs to assess this using your blood tests and imaging, because not all fatty livers are the same. Any weight loss treatment should take this diagnosis into account from the first consultation.
- Are online clinics that prescribe Mounjaro or Wegovy in Spain reliable, and how can I tell a trustworthy one?
- Some online clinics are reliable and others aren't. A trustworthy one assesses you properly, tells you who the doctor is, gives you a prescription to collect the genuine medicine from a pharmacy and follows you up. Be wary if they sell you the drug without a prescription, if you don't know who is treating you or if they promise guaranteed results. The EMA has warned of falsified Ozempic pens in the EU.
- Can I buy Mounjaro or Wegovy without a prescription in Spain?
- No. In Spain, both semaglutide and tirzepatide are prescription-only medicines. When they are sold online without a prescription, there is a risk that the product is falsified or unregistered, as cases involving illegal online pharmacies have shown.
